Cerner Corporation, a supplier of health information technology solutions, services, devices and hardware, has announced the launch of its Social Determinants Innovation Collaborative (SDIC), bringing over 30 health systems together to share the best practices and steps forward in addressing social determinants of health.

Defined by the Centers for Disease Control and Prevention (CDC) as “the circumstances in which people are born, grow up, live, work, and age,” social determinants of health (SDOH) have gained the attention of healthcare providers across the country. Social, environmental and economic factors, such as transportation, housing, nutrition and education level, can all have a big impact upon medical outcomes, resulting in a greater priority of SDOH from physicians as their finances are increasingly tied to quality or other “value-based” measures.

The group meets regularly to discuss their experiences solving challenges associated with social determinants of health as well as removing barriers to care in their communities.

Members of the collaborative include officials from the Children’s Hospital of Orange County, Los Angeles County Department of Health Services, and Marshall University School of Medicine in Huntington, W.Va.
